If you are not aware of the exact date, please state the approximate date of the incident giving the year and month.

Time of the incident

If you are not aware of the exact time, please state the approximate time.

Place of the incident

Please describe as accurately as possible where the incident occurred (country, county, city/municipality, city district, address). If possible, please specify the department/unit of the organisation involved in the incident.

Description of the incident

Please describe what happened in as much detail as possible such as your role, who were the other parties involved in the incident (names, positions), what is your estimation of the damage caused (also in EUR), what is the object at risk or what is causing the risk.

Origin of information

Please describe how you became aware of the information. If you were not present at the incident and you have learnt about it from other persons, please state the names and contacts of such persons.

Documents

Please upload the files evidencing the occurrence of the incident.

Additional information

If there are documents, photos or videos evidencing the occurrence of the incident, please provide the list of files and a brief description of the contents of such files.

The information provided is forwarded only to an independent auditor who ensures full confidentiality and anonymity of the whistleblower, analyzes the information received, and submits a summary of the report to Tallinn Water without any reference to the source of the information.
